Pagosa Ski Rental needs to get this form sent to us two weeks in advance in order to guarantee your equipment. 

We need one form per person (in the case of family reservations information may be combined
but we need the information for each skier or boarder).  Please fill out and fax to us! 
If you have given us two weeks notice, we'll have your equipment waiting for you!
